Breast Form (Prosthesis) Reimbursement Program

In Australia, permanent residents of holder of an eligible visa and are eligible for Medicare can claim up to $400 per prosthesis required if they have had surgery for breast cancer. Below is the first page of the form you need to fill out to claim the reimbursement which has the information about this process. I ask you to fill this out at your appointment and then I email it straight to Medicare with your receipt. 

If you have private health insurance please check with them first prior to your appointment to see if they will cover any of the cost of the breast form. You can also check if they will cover any cost of a mastectomy bra (Medicare do not cover this).

If you get financial help from DVA, you will need a letter from your GP confirming your clinical need for a breast prothesis and mastectomy bras. Bring the letter and your DVA Gold card to your appointment, and I will send off the claim, you won't pay for anything.
